How do I create a form?

Open any of the Business Solution apps (e.g. Transport Service).
Click on the Health Checks option.
Click on the Forms option to open the Forms page.
Click on the + add button.
Enter the form title and click on the Add new form button.
Your form is created.
Add or remove questions from the form as required and click on the Save button.
